New Patient Documentation
All initial patient in-processing documentation is available through the Patient Portal and must be completed prior to the initial consult. Portal access will be provided through email after scheduling and availability have been determined by the provider.

Documentation Updates

Patients who need to update existing records can  do so through the Patient Portal. After authentication select appropriate demographic and medical records -which need to be modified, and then alter existing information as needed. All alterations will be reviewed by your provider prior to finalization to ensure record accuracy and data integrity.

It is recommended that you arrive at least 15 minutes prior to your first scheduled appointment. For your convenience, a Patient Portal has been created that allows for secure and confidential interactions between yourself and the medical provider. 

In order for Dr. Theodosiadis to communicate with other physicians/therapists/schools, a release of information will need to be completed. Please have the name and contact information for those people involved in your care or your child’s care.

Please bring in a list of your current medications and dosages.

Prior to Initial Visit

Dr. Theodosiadis is considered an "out of network" provider. Our office will give you an invoice which you can submit to your insurance for reimbursement, on your own. Payment for services is expected in full and at the time of your appointment. Credit Card, Check and Cash payment are accepted.

Due to reimbursement challenges, as well as unreasonable restrictions insurers place on time allowed with patients, and frequency of visits, Sound Body & Mind will not direct, facilitate, or assist in the filing and collection of insurance claims.  We urge all new patients to inquire with their insurer regarding percentage of out of network coverage (OON) and the per-person OON deductible, so as to estimate out-of-pocket costs prior to the initial visit.
